Community Needs Assessment Survey

To gain a better understanding of the strengths and resources available in our communities that assist with providing for the needs of children, youth, and adults in our service areas, we are conducting a Community Needs Assessment Survey, October 27th, 2023 thru December 1st, 2023.

Surveys are an important communication tool that provide individuals in a community a pathway to give not-for-profit organizations insight as to how best to develop meaningful programs that positively impact the residents of their community.  By taking the time to answer a community needs assessment survey the participant is giving valuable feedback to organizations who are looking to improve upon necessary services such as infrastructure, healthcare resources, access to support systems addressing categories such as housing/food security/utility assistance/and more!

Social Determinants of Health, SDH, are the non-medical factors that influence health outcomes, and include a community’s access to:

  • Economic Stability
  • Social & Community Context
  • Neighborhood & Built Environment
  • Health Care & Quality
  • Education Access & Quality

Factors such as limited access to safe and affordable housing, food security, employment opportunity that generates an income above the poverty line, early childhood development program opportunities, access to quality health care services, and more, are all examples of social determinants of health.
