Our Sliding Fee Discount Program helps make health services more affordable for individuals and families based on income and household size.

As a designated Federally Qualified Health Center, our purpose is simple: ensure individuals and families can receive high quality primary care close to home, regardless of their insurance status or ability to pay.

What is the Sliding Fee Discount Program?

The Sliding Fee Discount Program at Southern West Virginia Health System offers the opportunity for our established patients to receive discounted healthcare services within our network of Health Centers.

The program is not designed to offset the cost of any medical tests or procedures for patients receiving their primary care from outside of our Health Center network.

Who is Eligible for the Sliding Fee Discount Program?

Any family or individual whose income meets financial guidelines is eligible for this discount service. Eligibility is based on total household income and family size. Even if you carry health insurance, you may still be eligible for out-of-pocket costs.

What Services Are Covered?

The program applies to most services, including:

  • Medical office visits
  • X-Ray/EKG fees
  • In-house lab fees
  • Behavioral health
  • Preventative dental visits
  • Pharmacy prescription medications
  • Chronic care management
  • Community health integration

Additionally, the program also applies to select pulmonary rehabilitation services, including:

  • Pulmonary function tests
  • 6-minute walk tests
  • Pulmonary rehabilitation initial intake
  • Individual pulmonary rehabilitation sessions
  • Group pulmonary rehabilitation services

How to Apply

Applying is simple and confidential.

  1. Download and complete the Sliding Fee Application form here OR request a paper form in-person from any Southern West Virginia Health System health center.
  2. Provide proof of income. This can include:
    • Recent pay stubs
    • Tax return
    • Social security or retirement benefits
    • And more; a list of acceptable proof of income is provided on the Siding Fee Application Form.
  3. Submit your application to the patient service representative who registers you for your appointment.
