Patient Experience Survey

At Southern West Virginia Health System, your feedback helps us improve the care and services we provide. Each fall and spring, we invite patients to participate in our Patient Experience Survey to share their thoughts about their recent visit.

The survey helps us understand what we are doing well and where we can improve. Your responses help guide decisions about patient services, communication, and health center operations so we can continue providing high quality, patient centered care.

The survey only takes a few minutes to complete, and all responses are confidential and anonymous.

We are a Health Center Program grantee under 42 U.S.C, 254b, and a deemed Public Health Service Employee under 42 U.S.C. 233(g)-(n). Southern West Virginia Health System is a Federal Tort Claims Act (FTCA) deemed facilities, meaning all malpractice claims are subject to federal procedural law. Our health centers serve all patients regardless of ability to pay.
