Southern West Virginia Health System is proud to be recognized as a Patient-Centered Medical Home (PCMH) by the National Committee for Quality Assurance (NCQA). PCMH recognition is a nationally recognized standard for primary care organizations that are committed to coordinated, patient-focused healthcare.
The Patient-Centered Medical Home model emphasizes stronger relationships between patients and their care teams, better coordination across services, and a
continued focus on improving quality of care. Health centers that achieve this recognition are evaluated against rigorous national standards that prioritize prevention, chronic disease management, care coordination, and continuous quality improvement.

Advancing Health Information Technology for Quality Recognition
Following a 2025 site-audit, Southern West Virginia Health System earned the Advancing Health Information Technology (HIT) for Quality Badge from the Health Resources and Services Administration (HRSA).
This recognition reflects health centers that effectively use technology to strengthen patient engagement and improve the delivery of care. It reflects our commitment
to modern healthcare systems that make it easier for patients and providers to connect, share information, and make informed care decisions.
To receive this recognition, health centers must demonstrate strong performance in several key areas, including:
- Implementation of an electronic health record (EHR) system
- Availability of telehealth services
- Secure electronic exchange of clinical information with other healthcare providers
- Patient engagement through health information technology
- Collection of data related to social risk factors that affect patient health

Certification of Compliance – 2025 Operational Site Visit
Southern West Virginia Health System earned the Operational Site Visit badge following a comprehensive review conducted in 2025. The Operational Site Visit, completed by the Health Resources and Services Administration on a routine three year cycle, evaluates health centers against rigorous national standards for patient care, safety, governance, financial stewardship, and day to day operations.
Achieving this recognition confirms full compliance across all required program areas. For patients and the communities we serve, it reinforces confidence that care is delivered by an organization that is accountable, well managed, and committed to quality, continuous improvement, and responsible use of resources.
