Seven SWVHS Locations Receive PCMH Recognition

Southern West Virginia Health System is proud to announce that several of our health centers have earned Patient Centered Medical Home (PCMH) recognition from the National Committee for Quality Assurance. This national recognition highlights health centers that are committed to providing high quality, coordinated, and patient focused care.

PCMH recognition reflects a care model that places patients at the center of the health care experience. Care teams work collaboratively to ensure patients receive the right care, at the right time, with strong communication between providers and support services.

What this means for our patients

Patients receiving care at PCMH recognized sites benefit from a more coordinated and proactive approach to health care. This includes:

• Care teams that work together to coordinate services across providers
• Greater focus on prevention and management of chronic conditions
• Easier communication, care coordination, and follow up
